Open enrollment typically runs November 1 through December 15. Outside that window, you may qualify for a Special Enrollment Period if you lose other coverage, get married, have a baby, or experience certain life events. Your advisor can confirm your eligibility.
The deductible is what you pay before your plan starts sharing costs for most services. The out-of-pocket maximum is the most you'll ever pay in a plan year — after that, the plan covers 100% of covered services.
It depends on the plan. HMO plans require you to stay in-network. PPO plans give you more flexibility. Your advisor will help you confirm that your current providers are in-network before you choose a plan.
